There are two ways to register for your child's CPAL drama session. If these methods don't work for you, call Donna Roth at ????. The first is to attend the CPAL registration open house. The Fall 2009 CPAL Open House will take place at the Lakota Family YMCA on Yankee Road on Saturday, September 19th from 10:00 am - noon. At the open house we will have all the necessary forms available to be filled out. The open house is a drop-in type event where parents and children can ask questions and fill out the necessary paperwork. PLEASE SPECIFY WHETHER YOU WOULD PREFER THE MONDAY EVENING CLASS OR THE TUESDAY EVENING CLASS. The second way to register is to use our online process. 1, Download the two forms below. There is a .doc, Microsoft Word version of each form and a .rtf version to choose from. If you have Microsoft Word on your computer, use the .doc versions of the forms. The .rtf version of the forms should be able to be opened by almost all word processing software. 2, Fill out both forms and save them under the same file extention (.doc or .rtf) as you you downloaded them under. 3, Send an Email to CPALakota@hotmail.com with the two forms attached. 4, On the first day of class, you will need to SIGN the medical form and pay for the session.
